Performance Evaluation

Katie has been working at her job as a nurse assistant at Fairhaven Care Center for three months. Today is the end of her probationary period, and she will be receiving a performance evaluation.

During her three-month probationary period, Katie has received an orientation to the care center and attended training classes in emergency and safety protocols. She has also worked with residents. During her orientation, Katie received an employee manual that listed the areas she would be evaluated on:

  • Quality of Care—Treat residents with empathy and concern. Identify and treat resident needs.

  • Teamwork—Work cooperatively with teammates. Resolve conflicts respectfully.

  • Knowledge of Policy and Procedures—Know and follow policy and procedures.

  • Customer Service—Have positive relationships with residents and their families.

  • Attendance—Be at work on time and ready to work. Take breaks only when assigned.

Katie feels she has been doing a good job. Her residents have told her that they like her. She gets along well with her co-workers and is always ready to help out. Her supervisor has been helpful whenever Katie has had a question about a procedure and has even complimented Katie on her work and attitude.

Katie is positive she will get a good review and become a permanent employee at Fairhaven Care Center.
